Community Health and Wellness Center of Greater Torrington (CHWC) is a Federally Qualified Health Center (FQHC) in the northwest corner of Connecticut, in a rural area 21 miles from Waterbury and 25 miles from Hartford. CHWC offers comprehensive primary medical care, women’s health care, dental and behavioral health services, as well as HIV and nutrition care to the uninsured and underinsured population of Litchfield County.
In support of CHWC’s efforts to convert it’s delivery model to comport with a Patient Centered Medical Home (PCMH) designation, including being recognized by the State of Connecticut as a service provider for substance abuse and the Suboxone treatment program, PCDC provided a $1 million loan from its Transformation Fund, to finance cashflow shortfalls due to a change of the center’s Electronic Health Records (EHR) platform, as well as quality improvement through strategic hirings. The initial loan was made in 2017 and recapitalized the facility in 2022.
